“Let him grow up first, we neuter after six months” — as an owner and a breeder, I heard that in almost every veterinary practice I walked into. And online, the exact opposite: “So early? That's cruelty, the cat won't develop properly!”. Meanwhile shelters have been routinely neutering kittens at eight to twelve weeks for thirty years, and breeders increasingly hand kittens over to their new homes already spayed or castrated. Who is right?
I decided to check the literature. It turns out the question was settled in research a long time ago — clinical practice in many places is simply decades behind. Here is what the hard data show.
Three terms worth telling apart
Three terms circulate in the literature and are frequently confused:
- Traditional-age gonadectomy (TAG) — surgery at 6–8 months or later. Still the most common practice in Polish clinics.
- Prepubertal gonadectomy (PPG) — before sexual maturity, in practice usually before 5.5 months of age.
- Early-age neutering (EAN) — at 6–16 weeks, often before the full course of standard vaccinations. The shelter standard, and that of many catteries.
When I write about “early neutering” below, I mean procedures performed before the first oestrus in females and before mature spermatogenesis in males — PPG and EAN taken together.

In 2016 the Veterinary Task Force on Feline Sterilization issued an unambiguous position: given the known benefits of sterilisation and the absence of evidence for harm related to the age at which it is performed, cats not intended for breeding should be neutered by 5 months of age. The recommendation has been endorsed by the AVMA, AAHA, AAFP and the Association of Shelter Veterinarians, among others. This is not a shelter invention — it is the current standard of veterinary medicine.
Why do we neuter at all?
Before we get into the argument about age, it is worth recalling what the procedure is for.
Lifespan
Kent and colleagues (2022) analysed thirty years of necropsies at the University of California, Davis veterinary hospital — 3,108 cats. Excluding animals that did not reach one year of age, the median ages at death were:
- intact females — 4.68 years, spayed females — 10.48 years;
- intact males — 3.67 years, neutered males — 9.84 years.

Honestly about those numbers: this is not the difference the surgery makes
It is tempting to say “neutering adds five to seven years to a cat's life”. That would be an overreach. The Davis study covers cats that ended up on a necropsy table at a referral hospital — Kent himself concedes it is “a bit of a skewed population”. Intact cats in that sample die young largely because they go outdoors more, fight more and are hit by cars more, not because the absence of surgery directly shortens life.
A better measure comes from Teng and colleagues (2024), who analysed 7,936 deaths in the UK VetCompass database. After adjusting for sex, breed and bodyweight, neutered cats lived on average 1.07 years longer (95% CI 0.72–1.42). The risk of death before the third year of life, however, was 4.29 times higher in intact cats (95% CI 3.72–4.95) — and it is that figure, not the necropsy median, that describes the real scale of the effect. Survival tables from the Banfield clinic database point the same way (Montoya et al. 2023).
Protection against mammary carcinoma
For me this is the strongest health argument in queens — because there is a clear window here, and it cannot be reopened.
Overley and colleagues (2005) ran a case-control study on 308 cases of mammary carcinoma and 400 control cats:
- spayed before 6 months of age — 91% risk reduction (OR 0.09; 95% CI 0.03–0.24);
- spayed before 12 months — 86% reduction (OR 0.14; 95% CI 0.06–0.34);
- spayed after the second year — no measurable protection.
In the same study intact queens were significantly overrepresented among the affected animals (OR 2.7; 95% CI 1.4–5.3). An older and much smaller study by Dorn and colleagues (1968) put the risk in intact queens at roughly sevenfold — a figure often quoted, but based on just 21 tumour cases, so I would treat it as a historical reference point rather than a precise estimate.
Why the window matters, and not simply the surgery
Exposure to endogenous oestrogens and progestagens across successive oestrous cycles promotes carcinogenesis in the mammary epithelium. Spaying before the first oestrus removes the source of those hormones before the tissue has a chance to respond to them. After the second year the process is already under way, and removing the ovaries does not reverse it. That is why the difference between surgery in the fifth month and in the twenty-fifth is so large — even though in both cases the cat ends up spayed.
The remaining benefits
In queens, spaying eliminates the risk of pyometra — a potentially fatal uterine infection — as well as vaginal hyperplasia and ovarian tumours and cysts. In males, castration removes the risk of testicular cancer.
On behaviour, Stubbs and colleagues (1996), in a randomised study with a control group, recorded significantly more intraspecific aggression, less affection towards people and stronger development of secondary sexual characteristics in intact cats. Spain and colleagues (2004), in a cohort of 1,660 cats, found that those neutered before 5.5 months had fewer fight abscesses, less aggression towards veterinarians, fewer sexual behaviours and less urine spraying — along with less asthma, gingivitis and hyperactivity.
The one downside in the same paper: a slightly greater tendency to shyness and hiding. A subtle effect, clinically insignificant, and correctable through socialisation.

The objection I hear most often: an early-neutered male will have a narrower urethra and will block more readily. This was tested radiographically thirty years ago.
Root and colleagues (1996) compared cats neutered at 7 weeks, at 7 months and left intact, measuring the urethra at 22 months of age:
- diameter of the preprostatic segment in males — no significant difference (p = 0.13);
- diameter of the penile segment — no difference (p > 0.25).
The one genuine anatomical difference: in males neutered before maturity the penis may not fully extrude from the prepuce. For a cat that will never breed, this carries no health consequence.
Sampaio and colleagues (2022) went a step further and looked at 84 male cats presenting at a clinic with urethral obstruction. The result ran counter to expectation: intact cats obstructed earlier (median 3.6 years) than those neutered prepubertally (5.7 years) or post-pubertally (5.5 years). Among intact cats, 87.5% had an episode before their fifth year, against roughly 55% in both neutered groups.
Myth two: obesity
“An early-neutered cat is bound to get fat” — almost as common. Here the answer is more nuanced, because the myth contains a grain of truth.
Foreman-Worsley and colleagues (2025) tracked bodyweight and body condition in the UK Bristol Cats cohort — 584 cats, 2,410 body condition measurements:
- bodyweight and body condition rise with age to roughly the ninth year and then decline — regardless of the age at neutering;
- no difference whatsoever between cats neutered at 4 months or younger, at 5 months and at 6 months;
- cats neutered later, between 7 and 12 months, gained more slowly and weighed slightly less throughout adult life.
So where does the myth come from? From the fact that after any neutering, at any age, something genuinely changes. Salt and colleagues (2023) showed across a large clinical database that food intake, bodyweight and fat mass all rise after surgery, most markedly in females. Obesity therefore comes from failing to adjust the ration afterwards, not from whether the procedure was done in the third or the sixth month of life.
The practical conclusion: after neutering, check body condition monthly and adjust the portion. Many cats need roughly a fifth fewer calories in the first months. How to tell that a cat has already put on weight, and what to do about it, I unpack in a separate piece on obesity.
Myth three: stunted growth
Will an early-neutered cat end up smaller? Paradoxically, the reverse is true.
Root and colleagues (1997) showed radiographically that neutering delays closure of the radial growth plates, producing a marginally greater final bone length. Neutered cats are therefore minimally taller. And — crucially for our question — there is no difference in this respect between surgery at 7 weeks and at 7 months.
Joints are where it gets more interesting. In dogs, early neutering has been linked to increased risk of degenerative joint disease. In cats the direction is reversed: in the Bristol Cats cohort, Maniaki and colleagues (2021) found that cats still intact at six months of age had 1.97 times higher odds (95% CI 1.26–3.07) of owner-reported mobility changes at six years of age. The other risk factors in the same model were previous trauma, outdoor access and being overweight.
What this study does not say
The outcome here was an owner-reported decline in mobility, not degenerative change confirmed radiographically. That distinction matters: owners were judging whether their cat jumped less well and moved less. Nor do we know the mechanism — the paper describes an association, it does not explain one. Any hormonal explanations you may read on the subject remain hypotheses.
Is surgery safe in a kitten?
This is where the real resistance lies. Many vets are wary of anaesthetising such small animals — and it was a justified concern, half a century ago.
Howe (1997) analysed complications in 775 cats and 1,213 dogs, split by age group: under 12 weeks, 12–23 weeks, and 24 weeks or older. The result:
- major complications: 2.9%, 3.2% and 3.0% — no significant difference between groups;
- minor complications: most frequent in the oldest group (7.8% versus 3.6% in the youngest);
- the overall complication rate was likewise highest in the oldest animals (10.8% versus 6.5%).
Porters and colleagues (2015) ran a prospective randomised trial on 800 shelter kittens aged 8–12 weeks, randomly assigned either to immediate neutering or to neutering deferred until 6–8 months, and followed them to 24 months of age. They found no significant differences in mortality, complications, lower urinary tract disease, urethral obstruction, lameness, fractures or dermatological conditions.
Howe and colleagues (2000) followed 263 shelter cats for a median of 37 months after surgery: prepubertal gonadectomy did not increase the incidence of infectious disease, behavioural problems or disorders in any body system — and the proportion of cats retained in their original adoptive household was the same in both groups.
The conditions that must be met
Safety in a kitten does not come from nowhere — it depends on several things the veterinarian has to attend to (Joyce and Yates 2011; Root Kustritz 2014):
- thermoregulation — kittens lose heat very quickly and need active warming;
- stable blood glucose — a short pre-operative fast, not one lasting many hours;
- surgical experience — the structures are small and the procedure takes practice;
- rigorous infection control, particularly in a shelter or cattery setting.
The accepted threshold is a minimum bodyweight of 1 kg, usually reached at around 12 weeks of age.
Early neutering in a cattery — and my own experience
For me as a breeder, early neutering is above all a way of protecting the breed from uncontrolled reproduction. The alternative is to release kittens under a contract obliging the new owner to arrange surgery — and here the data are unforgiving. The Cornell Feline Health Center reports that where sterilisation after adoption is mandatory, more than half of people fail to comply, even when a financial incentive is attached. In a survey of cat owners, 87% said their queen was spayed, but almost 90% of those admitted she had already had at least one litter beforehand. The most common explanation: they thought she was still too young to conceive. A queen can conceive as early as four months of age.

I can add an observation from my own practice, too. From the beginning I have neutered kittens at 12–14 weeks. Of roughly forty kittens that went through surgery with me, all recovered almost instantly — running and jumping the same day, as though the clinic visit had never happened. No complications at all. By contrast, when I spayed older or fully adult queens retiring from breeding — around five of them — a return to full form could take several days.
I know that is a handful of animals and an observation from a single cattery, not a study. But the difference in recovery between a young and an adult body was striking to me — and it points in exactly the direction Howe's data do: fewer complications in the younger animals, not the older ones.
When to wait
Early neutering is safe for most cats, but not for all:
- kittens intended for breeding — for obvious reasons;
- kittens under 1 kg or in poor general condition — we wait for improvement;
- kittens with congenital heart defects or unstable parameters — an individual decision;
- very large breeds (Maine Coon, Norwegian Forest Cat) — some breeders prefer 16–20 weeks for a greater margin of anaesthetic comfort. That is a practical preference, not a requirement arising from research.
Young age in itself is not a contraindication, provided the conditions above are met.
What to do if your vet refuses
Some practices still hold to the “minimum six months” rule, despite the absence of data supporting it. What you can do:
- Bring the literature. Spain et al. (2004), Porters et al. (2015) and the position of the Veterinary Task Force on Feline Sterilization. Many people simply do not know the consensus shifted decades ago.
- Look for a vet who works with a shelter. They perform pediatric neutering routinely and are practised at it.
- Or a vet who works with catteries. They know the procedure and what breed club regulations require.
- Accept a compromise. If your vet insists on the fifth or sixth month, that is still a good choice — you remain ahead of the first oestrus. Do not, however, postpone to the ninth or twelfth month, because by then most of the benefit is gone.
Summary
After thirty years of research — including randomised trials, prospective cohorts and large retrospective analyses — the picture is remarkably consistent.
Early neutering is safe: it does not increase the risk of major complications, does not narrow the urethra, does not stunt growth and does not by itself cause obesity. In queens, spaying before the first oestrus confers protection against mammary carcinoma that cannot be recovered later. And neutered cats live longer — realistically by about a year according to the best-controlled analysis, with a far lower risk of death in the first years of life.
If your vet suggests postponing surgery until your cat is nearly a year old, it is worth having the conversation — and if the conversation leads nowhere, worth finding someone whose practice rests on the current consensus. The timing of neutering is one of the more important health decisions of a cat's first year and, like any medical decision, it should follow from evidence rather than from what has always been said.

Will my cat end up smaller if neutered early?
Quite the opposite — marginally larger. Root, Johnston and Olson (1997) showed radiographically that in neutered cats the radial growth plates close later than in intact cats, giving slightly greater final bone length. This holds equally for neutering at 7 weeks and at 7 months — there is no difference between them. The effect is small and clinically irrelevant, but it neatly disposes of one of the most popular myths.
Does early neutering increase the risk of urethral obstruction in males?
No. This is the most persistent of veterinary myths. Root and colleagues (1996) measured urethral diameter radiographically in males neutered at 7 weeks, at 7 months and left intact — no significant differences in either the preprostatic or the penile segment. Sampaio and colleagues (2022) went further: among 84 males presenting with urethral obstruction, intact cats obstructed earlier (median 3.6 years) than those neutered prepubertally (5.7 years) or post-pubertally (5.5 years).
My vet says “minimum six months”. Where does that come from?
From an era when anaesthesia in young animals was far less safe than it is today. No study identifies six months as an optimal age — it is a customary number, not a calculated one. The 2016 consensus of the Veterinary Task Force on Feline Sterilization, endorsed by the AVMA, AAHA, AAFP and the Association of Shelter Veterinarians, says plainly: neuter by 5 months of age. In shelters and many catteries, 8–16 weeks is standard.
Will my cat get fat after neutering?
Not because of the surgery itself. Salt and colleagues (2023) showed across a large clinical database that food intake, bodyweight and fat mass all rise after neutering, most markedly in females. But Foreman-Worsley and colleagues (2025), following cats for years in the Bristol Cats cohort, found no difference in bodyweight or body condition between cats neutered at 4 months or younger, at 5 months and at 6 months. In other words: what matters is whether you adjust the portion after surgery, not which month it was done.
Are there any real downsides to early neutering?
One, and it is minor. Spain and colleagues (2004), in a cohort of 1,660 cats, recorded slightly more shyness and hiding among cats neutered before 5.5 months. In the same study that same group had fewer abscesses, less aggression towards veterinarians, fewer sexual behaviours, less urine spraying, and also less asthma, gingivitis and hyperactivity. The balance falls clearly in favour of the earlier procedure.
So what is the optimal age?
For an ordinary house cat: before the first oestrus, which in practice means by 5 months of age. That is the window in which protection against mammary carcinoma is greatest. For shelters, catteries and trap-neuter-return programmes: 8–16 weeks is safe and well studied. If your vet insists on 5–6 months, that is still a good choice. Postponing to 9–12 months forfeits most of the benefit.
